2024 – 25 Mini-Grant Application Instructions
This request for funding is for a maximum mini-grant award of up to $2,500.
Incomplete applications will not be considered.
Mini grants will be reviewed twice a year. To ensure timely review of your application, please submit grants by the following deadlines. Greater Plymouth CHNA steering committee members will review the grants semiannually.
Fall/Winter: Applications due December 31st (Funding notifications January)
Spring/Summer: Applications due April 30th (Funding notifications May)
Application Instructions
Complete and submit online application and budget sheet.
Both must be completed to be considered for a grant.Answer each question completely and concisely.
The Greater Plymouth CHNA Steering Committee may require you to
be available during the review of your application either in-person
or via telephone.
Eligibility
The proposed project must benefit one or more of the 11 communities in the Greater Plymouth Community Health Network Alliance catchment area. These communities include Carver, Duxbury, Halifax, Hanover, Hanson, Kingston, Marshfield, Pembroke, Plymouth, Plympton, Rockland
Special consideration will be given to innovative and creative approaches to address unmet areas of need. We ask that you characterize your grant as seeking “seed money” for a new program or a grow/expand project for an existing program.
Please note, for profit ventures will not be considered.
Projects funded in the previous calendar year will not be considered for funding.
Additional Requirements for grant recipients
Mini-grant funds must be spent within one year of the award notice.
A written outcomes project report is required. A template for the report can be found on our website (or otherwise made available).
Please plan to present your project outcomes at a GPCHNA meeting at the conclusion of the project. The CHNA 23 Steering Committee Chair or Vice Chair will reach out directly to schedule this presentation.
Events, materials, and other communications that are associated with the project funded by this mini-grant must acknowledge support from the Greater Plymouth CHNA. The following is suggested language: [Project Name] is funded [in part] by a mini-grant from the Greater Plymouth Community Health Network Alliance (CHNA).
In an effort to increase our social media capacity we encourage all awardees to tag @CHNA23 in any social media posts pertaining to the grant funded initiative. If you intend to utilize any other media to showcase projects made possible with CHNA 23 grant funding, including press releases, please forward these updates to chna23@gmail.com.
